Gastroenterological Medicine: Infectious Enteritis

Bowel Inflammation caused by the entry of pathogenic organism.

The disease where pathogenic organism orally entered the body, and infected with gut tract so that gut tract inflammation will cause vomiting, or diarrhea as symptoms. Pathogenic organism is, from major to minor, virus, bacteria or other parasitic insects.

Vomiting, diarrhea, fever, sometime blood feces

Symptoms are nausea, vomiting, diarrhea, fever and sometimes blood feces. Normally the symptom is in transient manner, and most of the case it will spontaneously cure, but depend on the condition of infected patient, the treatment using antibacterial agent may be necessary.

Assume pathogenic organism from food and travel history, so on.

Pathogenic organism can be assumed by knowing the detailed disease history, such as, food taken before symptom come up, progress of symptom, surroundings infection status, overseas travel history, and pre-existing disease. If necessary, pathogenic organism is diagnosed by adding the slops check, but most cases, it become less service by symptomatic therapy and positive rate is not high so that the treatment is applied for the specific pathogenic organism suspicious case or severely infected patient only.

Typical pathogenic organism and its curing method

Viruses are norovirus, rotavirus. Norovirus is contained in Oyster, and mainly spread around winter time. It’s incubation period is 1 thru 2 days. Symptom is a watery diarrhea, but it will be cured on two, three days thus it is relatively a light disease. Most of the case will become a less service by symptomatic therapy. But for those child or aged person, symptom of dehydration may occur so that if they cannot take the water, drip infusion or hospitalization may necessary. There is no needs for antibacterial agent. Bacterium are; Staphylococcus Aureus, Enterohemorrhagic Escherichia Coli, Vibrio Parahaemolyticus, Campylobacter, Salmonella, and Bacillus Dysenteries.

Enterohemorrhagic Escherichia Coli
Enterohemorrhagic Escherichia Coli exists in gut tract of cow or pig, and its incubation period is 4 to 8 days. Symptoms are severe abdominal pains, watery diarrhea, and later it may cause blood feces.
Campylobacter exists in Chicken or raw lever, and its incubation period is 2 to 10 days. Symptoms are mainly gastric pain, fever and diarrhea, but sometimes having pharyngeal pain or headache.
Most of Salmonella exists in egg or its processed food, and its incubation period is few hours to 2 days. Its symptoms are diarrhea, gastric pain, fever and vomiting, but sometimes fever become pervasive.
Bacillus Dysenteries
Bacillus Dysenteries occurs when taking food contaminated with feces or polluted hand/fingers after visiting developing countries in poor sanitation environment. Its incubation period is one to three days, and fever, gastric pain, sense of fatigue, blood feces containing pus, these symptoms appears out.

In case of bacterial enteritis, dysentery or typhoid most likely needs an antibacterial agent, but for other bacterial infection may require antibacterial agent depend on patient condition.
